To begin the process you must first call 811 to request an assessment. An Alberta Health Services Home Care Case Worker will then come out and assess the person needing care (either you or a loved one).
When the assessment is complete & if you receive approval you can now reach out to us and your Carelink support services inc Care Planner will take it from there!
